Barbara K. Prazak, MD
Medical Director
Director of Clinical Quality
Internal Medicine

Dr. Prazak is board certified in internal medicine, and has led the OCHS medical provider staff as Medical Director for seven years. As Medical Director she supervises the OCHS provider group and oversees recruitment, training and creation of up to date protocols necessary in today's changing health care environment. She obtained her Medical Degree from the University of Massachusetts Medical School, Worcester and completed an internal medicine residency at Mount Auburn Hospital, Cambridge. She has practiced as a board certified internist since 1983 and marked her 25th year on the OCHS medical team in 2014. She was honored with the Massachusetts League of Community Health Centers' Physician of the Year award in 2015. Dr. Prazak sees patients at all three health centers at OCHS. Dr. Prazak and her husband live in Wellfleet and have two grown children.

shay William Shay, MD

Dr. Shay is board certified in internal medicine. He obtained his medical degree from Albany College, interned at St. Vincent's Hospital and engaged in a large practice nearby. He is a member of the American College of Physicians and brings over 25 years of experience, with particular depth in providing primary care to people with HIV and those with more complicated medical conditions. He is committed to prevention and partnership with his patients. Dr. Shay is also Director of the Travel Clinic. He has had a home in Wellfleet since 1993, which he shares with his husband, two cats and a host of good friends.

Andrew Jorgensen, MD
Associate Medical Director
Internal Medicine & Pediatrics

Dr. Jorgensen is Associate Medical Director at Outer Cape Health Services, and is board certified in both Pediatrics and Internal Medicine. Dr. Jorgensen obtained his undergraduate degree with Honors from the University of California, Davis with a B.S. in Biochemistry in 1995, and received his Doctor of Medicine degree from St. Louis University School of Medicine in 1999 with a Distinction in Research. His Internal Medicine-Pediatrics Internship and Residency was completed in 2003 at the University of Texas, Houston. After his residency, Dr. Jorgensen practiced primary care in Texas, including as the founding partner of Houston Adult and Pediatric Medical Associates from 2003-2007, and was appointed Clinical Associate Professor at the University of Texas, Houston, from 2003-2007. Dr. Jorgensen was nominated for Physician of the Year in 2006 at the Children's Memorial Hermann Hospital in Texas. Prior to joining OCHS in 2015, Dr. Jorgensen was a practicing physician and Regional Medical Director at Cambridge Health Alliance. He lives year-round in Provincetown.

Meredith Goff, CNM

Certified Nurse Midwife
Meredith has been a clinician, lecturer, independent and private practice midwife. She obtained her diploma in Nursing from New England Deaconess Hospital, her BS in Nursing from the University of the State of New York, and MS with Specialty in Nurse-Midwifery from Columbia University in 1983. She is Certified by the American College of Nurse Midwives, and has received the regional Award for Excellence Nominee from the American College of Nurse Midwives.

Susan Flynn Roderick, CFNP, CPNP

Senior Nurse Practitioner
Director of the Teen Clinic

Susan obtained a BS in Nursing from the University of Rhode Island, her Pediatric NP and Family NP Certificate from Northeastern University. She has worked with many social service organizations, including Independence House, Provincetown Domestic Violence Intervention Office, Cape Cares, and Helping Our Women. She has provided health education classes to middle and high school students and started the Teen Health Clinic in 1992 to teach and empower young adults to act responsibly. Susan has worked at OCHS for over 3 decades.

Maryanne O'Toole, NP
Internal Medicine
Maryanne brings special expertise in geriatric medicine to OCHS. She earned her MSN in Gerontological Nursing from Massachusetts General Hospital in Boston. Ms. O'Toole's professional experience includes outpatient primary care, urology and short and long-term rehabilitation care. She considers patient empowerment her primary goal and feels strongly her background provides her with a holistic appreciation of her patient's individual health goals. She provides primary and urgent care fulltime in Provincetown and at Seashore Point Center.

Matthew Abar, PA-C
Family Medicine

Matthew has been a Physician Assistant-Certified at Outer Cape Health Services since 1988, providing primary care to a complete spectrum of patients, from infants to seniors. A Massachusetts native, he moved to New York to attend college, then started his medical training while working on whale watch vessels in Provincetown over several summers. That job inspired his return to Cape Cod after he earned his Physician Assistant Certificate at Albany Medical College, Union University, in Albany, New York. He appreciates working in a community health center where he is able to address everything from everyday health concerns to complex medical issues such as HIV/AIDS. Matthew is an avid cyclist and sailor.

Michael Henderson, OD

Dr. Henderson, optometrist, received his BS in Optometry from Indiana University in 1984 and his Doctor of Optometry from Indiana University in 1986. After practicing in Georgia for six years, he established an optometric practice in Bluffton, South Carolina for twenty-two years. Dr. Henderson joined OCHS in the fall of 2014 and has helped to establish the OCHS Optometry Services, open in the spring of 2015. Dr. Henderson lives on the Outer Cape.

Andrew Palmer, MD
Hand & Upper Extremity
Dr. Palmer received his MD from Upstate Medical Center in Syracuse, NY. He completed his General Surgery Internship and Orthopedic Surgery Residency at University of Michigan Affiliated Hospitals. He has been the recipient of many awards and grants for his investigational work on diseases of the hand and is the author of numerous articles pertaining to a wide range of medical and surgical topics. Dr. Palmer joined OCHS as a consultant in orthopedics of the upper extremity.

Christopher Rowley, MD, MPH
Director of HIV Medicine
Infectious Disease

Dr. Rowley is board certified in both Internal Medicine and Infectious Disease. He earned his MD from Brooklyn SUNY, magna cum laude, and his MPH from the Harvard School of Public Health. Dr. Rowley was a Clinical Fellow in Infectious Diseases at Beth Israel Deaconess Medical Center(BIDMC)and a Research Fellow in Molecular Biology of HIV at the Harvard School of Public Health. He has been appointed a Clinical Fellow in Infectious Diseases and an Instructor in Medicine at Harvard Medical School. Dr. Rowley is an Attending Physician in the Division of Infectious Diseases at BIDMC.

Andrew Wang, MD

Dr. Wang, board certified dermatologist, provides general dermatology services for adults and children of all ages at OCHS-Provincetown. Dr. Wang graduated from Princeton University in New Jersey with an A.B. in Ecology and Evolutionary Biology in 2003. He attended University of Medicine and Dentistry of New Jersey-Robert Wood Johnson Medical School and obtained an MD degree in 2009. He completed an Internal Medicine internship at Mount Auburn Hospital, Cambridge, MA, a Harvard University affiliated teaching hospital. He received his residency training in dermatology at Tufts Medical Center, where he was appointed Chief Resident in his final year. He is a recipient of the National Psoriasis Foundation Medical Dermatology Fellowship and the UMDNJ Robert Wood Johnson Research Scholar Fellowship. He has authored numerous book chapters and research papers in dermatology, specifically in the area of psoriasis.

Dikke Hansen, LICSW
Director of Behavioral Health

Marta "Dikke" Hansen, LICSW is the Director of Behavioral Health at OCHS. She is a licensed clinical social worker providing counseling for adults and children at OCHS-Provincetown. After graduating from college in 1975 with a Bachelor of Social Work from Augustana College, Sioux Falls, South Dakota, Ms. Hansen received her Master of Social Work degree from Michigan State University in 1981 and a Master's Certificate in Global Mental Health, Trauma and Recovery from Harvard University in 2008. Before joining OCHS in 2014, Ms. Hansen was the Clinic Director of the Family Counseling Center of the Massachusetts Society for the Prevention of Cruelty to Children (MSPCC) in Hyannis from 2011-2014. She has maintained a private counseling practice since 1992.

Peter Gherardi, MD

Dr. Gherardi is a board certified psychiatrist seeing adults for general psychiatric consultation in the OCHS–Provincetown health center. Dr. Gherardi graduated from Princeton University with a Bachelor of Arts degree in Philosophy and Psychology in 1983 and attended Tufts University School of Medicine, Boston MA, graduating with an MD degree in 1989. He completed a Medical Internship at St. Elizabeth’s Hospital, Boston, MA in 1990 and a Residency in Adult Psychiatry at the Massachusetts Mental Health Center, Boston, MA from 1990-1992. He completed a Fellowship in Child and Adolescent Psychiatry at the Massachusetts General-McLean Combined Program, Boston, MA from 1992-1994. Dr. Gherardi continues to maintain a private practice in the Boston area.

Daniel Wyatt, MD
Dr. Wyatt is an adult and child psychiatrist with over 20 years of both inpatient and outpatient experience. Dr. Wyatt received his Doctor of Medicine degree (MD) from the University Of Arkansas School for Medical Sciences. He completed a three-year fellowship in Child and Adolescent Psychiatry at the Yale Child Study Center in New Haven, CT, and was Director of the Acute Residential Treatment Program for Children and Adolescents at McLean Hospital. He currently serves as the Medical Director for the Mass Society for the Prevention of Cruelty to Children in Hyannis. Dr. Wyatt provides consultations for OCHS primary care patients in both Wellfleet and Provincetown health centers.

Amy Wolf, LICSW
Social Work
HIV Counseling
Amy is a graduate of Wesleyan University, where she obtained a BA with High Honors and was awarded the Walkley Prize for excellence in Psychology. She earned her MSW at Hunter College School of Social Work. She has worked as a Case Planner, Social Worker and Clinician, with special training in parent-child conflict mediation, as well as “Brief Systemic Therapy” with couples and families. She has spent time studying and working in both Kenya and the Netherlands.

Linda Linsinbigler, DMD

Dr. Linsinbigler is accepting new adult and pediatric dental patients at OCHS-Provincetown. Dr. Linsinbigler obtained her BS degree in Chemical Biology from St. Joseph’s University, Philadelphia and her MBS in Biomedical Sciences from the University of Medicine and Dentistry of New Jersey. She graduated magna cum laude from the Boston University Henry M. Goldman School of Dental Medicine with the Degree of Doctor of Dental Medicine (DMD) in May 2015. She was a member of the National Dental Honor Society while in dental school, as well as a volunteer for many community dental outreach projects in the Boston area. Dr. Linsinbigler is a member of the Massachusetts Dental Society. She enjoys traveling, the beach, and practicing Spanish.

Stanley Pimenta, DDS
Dr. Pimenta is a Doctor in Dental Surgery. He is a 2000 graduate from Estacio de Sa University School of Dental Medicine, Rio de Janeiro, Brazil. He moved to Massachusetts and has served in various dental positions from Worcester and Boston since April of 2001. Dr. Pimenta speaks English, Portuguese and Spanish.
John Treeful, RDH
Dental Hygienist

John received a BA in Spanish and his M.Ed. at the University of Phoenix in Arizona. He received his Certificate in Dental Hygiene at Massachusetts College of Pharmacy and Health Sciences in Boston, MA. Before joining OCHS, John served Outer Cape residents as a hygienist at a Wellfleet dental facility and at Martha’s Vineyard Hospital. He is a year-round resident of Provincetown.
Marian Lalevee Marian Lalevee, RD, CDE, LD
Medical Nutrition Therapy
Marian is a 30-year resident of the Cape with an equal number of years providing medical nutrition therapy and diabetes education to individuals and their families all over the Cape, assisting clients meet their weight, blood glucose, lipid and other health and wellness goals. After employment at Cape Cod Hospital for 28 years, she left to develop a private practice in Harwich Port, and joined OCHS in the fall of 2014. Ms. Lalevee obtained a Bachelor of Science in Food and Nutrition from the College of St. Elizabeth, NJ, a Master of Science in Foods and Nutrition from the University of North Carolina, and is registered with the Academy of Nutrition and Dietetics and is a certified diabetes educator and a licensed dietician in Massachusetts. Ms. Lalevee lives in the Lower Cape with her family.
