Barbara K. Prazak, MD
Medical Director
Director of Clinical Quality
Internal Medicine

Dr. Prazak is board certified in internal medicine, and has led the OCHS medical provider staff as Medical Director for seven years. As Medical Director she supervises the OCHS provider group and oversees recruitment, training and creation of up to date protocols necessary in today's changing health care environment. She obtained her Medical Degree from the University of Massachusetts Medical School, Worcester and completed an internal medicine residency at Mount Auburn Hospital, Cambridge. She has practiced as a board certified internist since 1983 and marked her 25th year on the OCHS medical team in 2014. She was honored with the Massachusetts League of Community Health Centers' Physician of the Year award in 2015. Dr. Prazak sees patients at all three health centers at OCHS. Dr. Prazak and her husband live in Wellfleet and have two grown children.

Matthew Libby, DO
Family Medicine

Dr. Libby is board certified in primary care family medicine. He completed his post-doctoral residency at the Lawrence Family Medicine residency program in Lawrence, MA which is nationally recognized as a top family medicine training program. Dr. Libby won the Sauter Award for Graduate with Distinction for possessing those qualities the faculty would seek in their family physician. Dr. Libby sees each patient as a whole person, and combines todays technology with careful listening to his patients in order to diagnose and treat those patients for injury and illness.

H. Todd Spencer, MD
Family Medicine

Dr. Spencer is board certified and former Medical Director of Mt. Ascutney Physican Practice in Windsor, VT, an affiliate of Dartmouth Hitchcock Medical Center. He will be practicing primary care medicine in Wellfleet and is accepting new patients. He received his BS degree from the University of Vermont and his MD degree from the Boston University School of Medicine, and for many years was an Adjunct Assistant Professor of Medicine at Dartmouth Medical School. He brings three decades of experience in primary care/internal medicine to OCHS, with a focus on Geriatric care. He lives on the Outer Cape with his family.

Meredith Goff, CNM
Director of Nursing
Director of Prenatal Care
Certified Nurse Midwife

Meredith has been a clinician, lecturer, independent and private practice midwife. She obtained her diploma in Nursing from New England Deaconess Hospital, her Bachelor of Science in Nursing from the University of the State of New York, and Master of Science with Specialty in Nurse-Midwifery from Columbia University in 1983. She is Certified by the American College of Nurse Midwives, and has received the regional Award for Excellence Nominee from the American College of Nurse Midwives.

Susan Flynn Roderick, CFNP, CPNP
Senior Nurse Practitioner Director of the Teen Clinic

Susan obtained a bachelor's degree in Nursing from the University of Rhode Island, and her Pediatric NP and Family NP Certificate from Northeastern University. She has worked with many social service organizations, including Independence House, Provincetown Domestic Violence Intervention Office, Cape Cares, and Helping Our Women. She has provided health education classes to middle and high school students and started the Teen Health Clinic in 1992 to teach and empower young adults to act responsibly. Susan has worked at OCHS for over three decades.

Anna DeLude, NP
Adult Gerontology

Anna is an adult gerontology-trained board certified Nurse Practitioner. She received her bachelor of arts in Political Science from Boston College in 2008. Ms. DeLude was a registered nurse at Cape Cod Hospital (2013), and on the cardiovascular unit of Metro-West Medical Center in Framingham, MA (2012-2013). She received her Masters of Science in Nursing as an Adult Gerontology Nurse Practitioner from the University of Massachusetts, Worcester Graduate School of Nursing in 2013. During her studies she was the recipient of the Federal Health Resources and Services Administration’s Advanced Nursing Education Expansion Scholarship and Community Service Award in 2013.

Ellen Dennehy, PA-C
Family Medicine
Ellen is a Physician Assistant-Certified (PA-C) providing primary care to a complete spectrum of patients, and especially enjoys caring for families and children. Ms. Dennehy graduated Cum Laude from Massachusetts College of Pharmacy and Health Sciences, and earned her BS degree from the Rochester Institute of Technology. She began her career as an ophthalmic photographer and enjoys Kayak racing and surfing the better Outer Cape breaks on a Long Board.

Gretchen Eckel, PA-C
Family Medicine
Gretchen practices family medicine and provides primary care for both children and adults. She graduated cum laude from the University of Rochester and earned her Master’s degree in Physician Assistant Studies from the Massachusetts College of Pharmacy & Health Sciences. She completed clinical rotations throughout New England healthcare facilities including Tufts Medical Center, Rhode Island Hospital and Outer Cape Health Services.

John Kalin, MD
Dr. Kalin received his medical degree from the University of Massachusetts Medical School and fellowships in Cardiovascular Disease and Clinical Electrophysiology. Dr. Kalin was the recipient of First Prize at the annual meeting of the New England Electrophysiology Society (NEEPS) Fellow’s Competition and is board certified in Internal Medicine and Cardiovascular Diseases and is a member of the American College of Cardiology. Dr. Kalin practices as a general cardiologist and as an electrophysiology specialist at Cardiovascular Consultants of Cape Cod in Hyannis.

Joseph Zibrak, MD
Pulmonary Medicine
Dr. Zibrak is board certified in Internal Medicine and Pulmonary Medicine. Dr. Zibrak also practices intensive care medicine as an active member of the medical staff of Beth Israel Deaconess Medical Center. He is an Assistant Professor of Medicine at Harvard Medical School and graduated cum laude from Colgate University, interned and completed a medical residency at the University of Michigan, and completed a Pulmonary Fellowship at Boston University School of Medicine. Dr Zibrak sees adult patients in pulmonary consultation year-round at the Wellfleet Health Center and has a home in Wellfleet.

Dikke Hansen, LICSW
Director of Behavioral Health

Marta "Dikke" Hansen, LICSW is the Director of Behavioral Health at OCHS. She is a licensed clinical social worker providing counseling for adults and children at OCHS-Provincetown. After graduating from college in 1975 with a Bachelor of Social Work from Augustana College, Sioux Falls, South Dakota, Ms. Hansen received her Master of Social Work degree from Michigan State University in 1981 and a Master's Certificate in Global Mental Health, Trauma and Recovery from Harvard University in 2008. Before joining OCHS in 2014, Ms. Hansen was the Clinic Director of the Family Counseling Center of the Massachusetts Society for the Prevention of Cruelty to Children (MSPCC) in Hyannis from 2011-2014. She has maintained a private counseling practice since 1992.

Peter Gherardi, MD

Dr. Gherardi is a board certified psychiatrist seeing adults for general psychiatric consultation in the OCHS–Provincetown health center. Dr. Gherardi graduated from Princeton University, NJ with a Bachelor of Arts degree in Philosophy and Psychology in 1983 and attended Tufts University School of Medicine, Boston MA, graduating with an MD degree in 1989. He completed a Medical Internship at St. Elizabeth’s Hospital, Boston, MA in 1990 and a Residency in Adult Psychiatry at the Massachusetts Mental Health Center, Boston, MA from 1990-1992. He completed a Fellowship in Child and Adolescent Psychiatry at the Massachusetts General-McLean Combined Program, Boston, MA from 1992-1994. Dr. Gherardi continues to maintain a private practice in the Boston area.

Daniel Wyatt, MD
Dr. Wyatt is an adult and child psychiatrist with over 20 years of both inpatient and outpatient experience. Dr. Wyatt received his medical degree from the University Of Arkansas School for Medical Sciences. He completed a three year Fellowship in Child and Adolescent Psychiatry at the Yale Child Study Center in New Haven, CT. and has held positions at McLean Hospital as Director of the Acute Residential Treatment Program for Children and Adolescents. He currently serves as the Medical Director for the Mass Society for the Prevention of Cruelty to Children in Hyannis. Dr Wyatt provides consultations for OCHS primary care patients in both Wellfleet and Provincetown health centers.

Susan Livingston, LICSW, MSW
Social Work
Susan obtained a bachelor's degree from Salve Regina College and MSW from the University of Connecticut plus advanced training in Dialectic Behavior Therapy, Critical Incident and Stress Management. She a clinician with experience in the full continuum of psychiatric services including emergency assessment, inpatient, residential, partial hospital, day treatment and intensive outpatient treatment settings.

Marian Lalevee Marian Lalevee, RD, CDE, LD
Medical Nutrition Therapy
Marian is a 30-year resident of the Cape with an equal number of years providing medical nutrition therapy and diabetes education to individuals and their families all over the Cape, assisting clients meet their weight, blood glucose, lipid and other health and wellness goals. After employment at Cape Cod Hospital for 28 years, she left to develop a private practice in Harwich Port, and joined OCHS in the fall of 2014. Ms. Lalevee obtained a Bachelor of Science in Food and Nutrition from the College of St Elizabeth, New Jersey, a Master of Science in Foods and Nutrition from the University of North Carolina, and is registered with the Academy of Nutrition and Dietetics and is a certified diabetes educator and a licensed dietician in Massachusetts. Ms. Lalevee lives in the Lower-Cape with her family.
