Out-of-Network health coverage can often result in higher costs to the patient. Visitors seeking walk-in service at our health centers, or urgent care in Provincetown, should seek preauthorization from their insurance providers to avoid a potential out-of-pocket expense for visits.

How Do I Apply for Health Insurance?

What documents do I need to apply for MassHealth?
To complete the MassHealth application, you should bring your last two pay stubs, or most recent tax return if you are self-employed, as well as photo identification and proof of citizenship for U.S. citizens.
