PLEASE NOTE: You will need to sign up for a MyChart account even if you had a previous patient portal account with Outer Cape Health Services before.
If you already have a MyChart account with another provider or organization, you will still have to create a separate Outer Cape Health Services MyChart account. Your full OCHS records will not completely transfer over until you have your first in-person visit at an OCHS health center after signing up for MyChart.

STEP 1: Get an activation code.
You can click the button below to request an activation code or ask at your next appointment.
Outer Cape Health Services will send you an activation code by text or email.
STEP 2: Create Your MyChart Account.
BY TEXT
1. For a text, tap the unique link to MyChart. PLEASE NOTE: the link expires in 45 days.
2. Create a username, password, and fill in your information. Make sure your username and password is something you will remember!
3. SUCCESS! You have created your MyChart account!
BY EMAIL
1. From your email, click the unique link to MyChart. PLEASE NOTE: the link expires in 45 days.

If you received an activation code in a letter or on your printed After Visit Summary, visit mychart.ochin.org/MyChartOCHS/accesscheck.asp to complete your sign-up.
STEP 3: Access MyChart from your personal computer or mobile phone.
FROM YOUR COMPUTER
• Visit the MyChart website at https://mychart.ochin.org/MyChartochs/
• Enter your username and password and click “Log In.”
FROM YOUR MOBILE PHONE
• Go to the app store on your phone and search for “MyChart”
• Install and open the app
• Select “Massachusetts”
• Scroll down and select “Ochin|MyChart”
• Enter your username and password and click “Log In.”
